Billing PolicyThis listing reflects current DMG contracts. However individual physician’s network participation may vary. In order to receive the highest level of benefits, please check with your health plan to verify physician participation.
Insurance PreparationsReview billing statements and contact Customer Service if you believe that your statement is not correct, need an explanation of the statement or can provide us with updated information. Contact Customer Service if you cannot pay your balance in full. For faster service, please give Customer Service your account number. Your first statement will be sent to you within 30 days after the date your service provided. The statement will identify what we have billed to insurance and what is your responsibility to pay. For our convenience we accept most major credit cards, personal checks or cash. Payment can be made at your doctor’s office or sent to the address below. Credit card payments may also be made by contacting Customer Service at (630) 942-7998.
When you contact Customer Service please have your account number ready. This will help Customer Service access your account more quickly. Your account number is located in the upper left hand corner of your billing statement. Cancellation of AppointmentsOtolaryngology, Head & Neck Surgery of DuPage Medical Group requires a 24 hour notice for missed appointments. Note: Do not cancel appointment via email. To cancel an appointment, please call the office directly.
